VitalCare for Kids (EPSDT) Private Duty Nursing (PDN) Over-Cap Form
Effective immediately, providers are no longer required to complete the MaineCare Treatment Services Justification for Exceeding the PDN/PCS Cap Section 94, Prevention, Health Promotion, and Optional Treatment Services Form in the member’s medical record when the PDN cap is exceeded for members under age 21.
The form is now obsolete due to updated processes related to VitalCare for Kids within the MaineCare Prior Authorization Unit. We also removed it from the Health PAS Online Portal.
