Survey for HCBS Providers and Interested Parties of Clinical, Therapeutic, Supportive Skills Building, Assistive Technology, Care Coordination and Personal Support Services
Over the last several weeks, the Department with its vendor Public Consulting Group LLC (PCG), hosted several interested party engagement meetings to gather important feedback that will inform the rate determination process for certain services under Sections 12, 18, 19, 20, 21, 29, 40, 96, and 102. All presented materials to date can be found on the Rate System Reform webpage.
To further support this rate determination process, the Department and PCG developed a survey to collect information from providers and interested parties including program costs and service utilization. It is important that there is adequate participation from the provider community. This information will be key in the development of recommendations related to reimbursement rates and/or payment models.

- Please complete the survey by Friday November 1, 2024. We have uploaded a recorded training on how to access, navigate and complete the survey on the Rate Reform webpage.
- To ensure the integrity of the results, please coordinate within your organization to ensure that only one survey per organization is submitted.
The survey is hosted through a HIPAA-compliant platform to ensure the security of your data and information. Survey responses will be kept confidential and provided in a de-identified format to ensure data integrity. No provider-specific information will be shared or published.
