Sleep Apnea Prior Authorization (PA) Sheet Now Available, CR 117762
Effective July 31, 2023, OMS added the Sleep Apnea PA Criteria Sheet to the Health PAS Online Portal for dental services.
The PA form is available by logging into your Trading Partner Account in the secure section of the Health PAS Online Portal and by following this pathway: MaineCare Information>Prior Authorizations>Manual, Forms, and Criteria Sheets, under Med Supplies/DME Section 60.
