MaineCare is once again inviting you to submit presentation proposals this year!

Presenting at the MaineCare DSR Annual Forums, either individually or as part of a team, is a wonderful way to share expertise, experience, and enthusiasm for your work. The forum's community-generated program will showcase future directions, best practices, stories of successful collaborations, lessons learned, and solutions to community-wide issues within various program tracks focus areas.

Whether you are a first-time presenter or a veteran public speaker, we welcome your submission!

Ready to get started? Please review the Steps and Guidelines for Submission carefully before you begin working on your proposal and be sure to submit it by the deadline: September 12, 2025. You can download the Proposal Template for advanced work and/or collaboration.

Forum attendees will include health care providers working within the following initiatives/models:

  • Accountable Communities: contracted entities have accountability for costs and quality for an attributed population through a shared savings model
  • Behavioral Health Home (BHH): provides team-based, whole-person care coordination services to individuals with serious and persistent mental illness and children with serious emotional disturbance
  • Certified Community Behavioral Health Clinics (CCBHCs):  provides a broad range of community behavioral health and focus on coordinated care and use of evidence-based practices for individuals across their lifespan, regardless of their ability to pay
  • Community Care Team (CCT): provides team-based, whole-person care coordination services to individuals with chronic conditions and complex care management and coordination needs
  • Housing Outreach and Member Engagement (HOME): provides team-based, whole-person care coordination services to individuals with long-term homelessness
  • MaineMOM: provides team-based, whole-person care coordination, perinatal and opioid treatment services to pregnant individuals with Opioid Use Disorder (OUD)
  • Opioid Health Home (OHH): provides team-based, whole-person care coordination services to individuals with OUD
  • Primary Care Plus (PCPlus): provides primary care services through a population-based payment model, while encouraging a team-based approach and integration with behavioral health care
