Updated 340B Agreement Form
The 340B Agreement Form was updated and posted to the Health PAS Online Portal. When renewing your 340B Agreement, providers should check for the most current version as older versions are no longer valid. Using an older version will result in the form or an enrollment maintenance case being rejected.
