​Primary Care Plus (PCPlus) and Community Health Worker Overview Webinar​

MaineCare, in collaboration with Maine Center for Disease Control, is hosting a webinar focusing on the collaboration between PCPlus practices and Community Health Workers (CHWs) or Community-Based Organizations (CBOs) employing CHWs. The goal of this webinar is to:  

• Provide a brief overview of the CHW workforce in Maine   

• Provide an overview of incentives within PCPlus to increase access to CHW services  

• Offer resources to both primary care practices and CHWs or CBOs who have an interest in learning more about partnering to support patients in their community  

• Hear from community partners who will share knowledge on:  

• CHW training and apprenticeship opportunities  

• Resources to find CHW services or CBOs who offer CHW services  

• Technical assistance opportunities   

• Peer sharing on related efforts

This webinar is open to all interested parties. Consider attending if one or more of the following applies to you:   

• You are part of an organization or practice interested in learning more about PCPlus and the collaboration with CHWs or CBOs 

• You are part of a participating PCPlus practice.   

• You belong to a primary care practice interested in joining PCPlus 

• You are a CHW 

• You belong to a CBO that offers CHW services
