Deadline Extended: Apply to be a Presenter at the 2024 MaineCare Delivery System Reform Forums

MaineCare’s Delivery System Reform (DSR) Annual Forums will be held on:

  • September 16, 2024, in Auburn, ME
  • September 24, 2024, in Orono, ME

This year, MaineCare is turning the tables and wants to hear from YOU! Consider applying to be a presenter. The deadline to apply has been extended! All proposals must be submitted by the new deadline of June 24, 2024.

Presenting at the MaineCare DSR Annual Forums is a wonderful way to share expertise, experiences, and excitement for new ideas. The forum's community-generated program will showcase best practices, stories of successful collaborations, lessons learned, and solutions to community-wide issues within various program tracks.

Whether you are a first-time presenter or a veteran public speaker, we welcome your submission! Please read the Call for Proposal Information Page carefully before you begin work on your proposal.

Forum attendees will include health care providers working within the following initiatives/models:

Accountable Communities: contracted entities have accountability for costs and quality for an attributed population through a shared savings model.

Behavioral Health Home (BHH): provides team-based, whole-person care coordination services to individuals with Serious and Persistent Mental Illness and children with Serious Emotional Disturbance

Community Care Team (CCT): provides team-based, whole-person care coordination services to individuals with chronic conditions and complex care management and coordination needs

Housing Outreach and Member Engagement (HOME): provides team-based, whole-person care coordination services to individuals with long-term homelessness

MaineMOM: provides team-based, whole-person care coordination, perinatal and opioid treatment services to pregnant individuals with Opioid Use Disorder

Opioid Health Home (OHH): provides team-based, whole-person care coordination services to individuals with Opioid Use Disorder

Primary Care Plus (PCPlus): provides primary care services through a population-based payment model, while encouraging a team-based approach and integration with behavioral health care.
