The Minimum Data Set (MDS) Submission Management System (SMS) will be down from 5:00 pm on March 30, 2022 until 8:00 am April 4, 2022.

On April 4, 2022, the SMS web address will change to: https://www.maine.gov/dhhs/sms/. NF, RCF, and AFCH providers must login to the SMS secure system with the same username and password used previously to submit MDS-RCA and MDS-ALS assessments and access the final State validation reports for MDS 3.0.
