This applies to your EMS, EMD, and Ambulance Operator License
To change your mailing address, email address, or phone number:
- Go to the Maine EMS eLicensing Portal
- Login to your account with your username and password (remember that if you enter the wrong information 3 times you will locked out. Try using “Forgot Username or Forgot Password” which will email the information to the email address on file for your account)
- Under “My Account” on the left, Click on "Profile"
- Under "Demographics", edit any contact info as needed, then click "Save" at the bottom.
- Users can only edit address, email and phone numbers.
To change your name:
- Go to the Maine EMS eLicensing Portal
- Login to your account with your username and password (remember that if you enter the wrong information 3 times you will locked out. Try using “Forgot Username or Forgot Password” which will email the information to the email address on file for your account)
- Under “My Account” on the left, Click "Applications" and scroll down to “Request for Legal Change of Name” and click the “Apply Now” button
- Carefully read the document, then Click the Acknowledgement and then “Save and Continue” button
- Change your name as appropriate and ensure correct spelling
- In Section 3. Attach a scanned or electronic document proving the legal change of name (such as a marriage certificate or other court documents) by clicking on “Upload File” and locating the document on your computer
- Confirm the change in Section 4 by entering your login password
- Fill in the date of application (in MM/DD/YYYY format)
- Click Submit, then click Yes on the pop up box to confirm submission
- This will submit the application and you can log out. If any questions arise, one of our licensing agents will contact you. Applications are processed in the order they are received and may take 7-10 business days to process. You will receive an email once the process is complete.
