Number of Older Adult Individuals Receiving State-funded, long-term Services and Supports – Aggregation: Average

  • Metric Explanation: A count of older adult individuals receiving state-funded long-term services and supports community programs, which includes home-based care (Section 63) and homemaker services (Section 69).

All Home and Community-Based Services (HCBS)-receiving Individuals Served – Aggregation: Average

  • Metric Explanation: A count of individuals receiving long term services and supports via Medicaid 1915c Home and Community Based Waiver Service programs per MaineCare Benefits Manual (Sections 18, 19, 20, 21, 29).

    Each waiver serves adults with disabilities, which includes older adults or persons with physical disabilities and adults with intellectual or developmental disabilities, such as autism spectrum disorders, traumatic/acquired brain injury, or other related conditions.

Number of Public Guardians/Clients – Aggregation: Average

  • Metric Explanation: The number of adults in public guardianship and/or conservatorship with Adult Protective Services (APS).

Number of Older Adults using Nutritional Services – Aggregation: Average

  • Metric Explanation: The number of older adults receiving Nutritional Services including congregate meals, home delivered meals, and other nutritional support.

Number of Individuals with Intellectual and Developmental Disabilities (IDD) using self-directed MaineCare benefits Home and Community Based Service (HCBS) Waiver Options – Aggregation: Average

  • Metric Explanation: The number of adults with Intellectual and Developmental Disabilities (IDD) utilizing self-directed MaineCare Benefits Manual Sections 18, 20 and 29 Home and Community Based Services (HCBS) waiver options.

    Self-directed services include: financial management, supports brokerage, individual goods and services, home support, community membership, home accessibility adaptations and assistive technology.

Number of Adult Individuals with Intellectual and Developmental Disabilities (IDD) receiving Section 21 & 29 Home and Community Based Service (HCBS) Waiver Options – Aggregation: Average

  • Metric Explanation: The number of adult individuals with Intellectual and Developmental Disabilities (IDD) receiving Section 21 and Section 29 Home and Community Based Services (HCBS) residing in community-based placements.

    Placements include agency-owned group homes (T2016), shared living (S5140), and in-home supports (T2017).

Number of Individuals with Intellectual and Developmental Disabilities (IDD) using Career Planning Services – Aggregation: Average

  • Metric Explanation: The number of adult individuals with Intellectual and Developmental Disabilities (IDD) utilizing career planning services (T2015) in Sections 18, 20, 21, and 29 of the MaineCare Benefits Manual.

    Career planning assists the waiver member with developing an individualized plan for competitive employment.
