Applicants for Permanent Physician Licensure:

IMPORTANT:  Effective 02/01/2022, the Osteopathic Board requires that all licensees to complete FSMB's Federation Credentials Verification Service.   

To apply:  Click on the 'FSMB' link below and follow the instructions to apply through the Maine Board of Osteopathic Licensure.   Please be sure to thoroughly review the UA instructions and checklist.

Check your application status online: 

License Applicants can check their status online by going to the Board's website, click on LICENSE OPTIONS  then CHECK THE STATUS OF AN APPLICATION.  Follow the log in instructions from there.  

NOTE:  When you receive a message stating that there are no possible transactions available, your application requirements have been met and your application will be presented for review at the Board's next scheduled meeting. 

Note:  The Board has ended its requirement for verification of other state licenses submitted on their behalf when applying for a Maine D.O. license. License verification is now handled by Board staff with one exception: In cases where an applicant was the subject of a Board action, he or she must provide all information related to that action. 

The fee for a permanent license application is $350 (a minimal fee will be charged by the processing company). Checks are accepted but online payment at the time of Addendum completion is preferred

Addendums not submitted online must be signed by the applicant and the other requested information must be provided before mailing it to the Board office.  The online checklist will not be available when an applicant chooses this option.  NOTE:  Regardless of signed authorizations

***STAFF CANNOT PROVIDE APPLICATION STATUS INFORMATION TO THIRD PARTIES***   

Applicants must use the Affidavit & Reference forms for this Board (reference letters are not accepted).  Per the instructions on the UA Addendum, pre-filled forms and/or forms submitted with stamped or scanned signatures are not accepted; references must be submitted by the writer, not by the applicant or a third party.

IMPORTANT: The Board does not accept electronically notarized documents. Applicants must be in the direct presence of the notary.  If not completed correctly, the Affidavit will need to be returned.

Click FSMB above to complete and submit your Uniform Application. Please be sure to check off for it to be sent to the Maine Board of Osteopathic Licensure. The FSMB charges a $60 fee to submit the application. Once you have completed and submitted the online application, please complete the DO Board Affidavit.pdf and submit it to either the Board office or your Program Coordinator (check with them for their preference).  The day following submission of your UA, you will receive an e-mail from noreply@maine.gov which will contain a link for you to complete your online Addendum; please complete this as soon as possible, following all instructions. 

Because we are asked often, please check with your Program Coordinator to determine whether or not your Program will reimburse you the $60 online application fee. 

Affidavits must be complete and notarized as instructed.
