HOW DO I APPLY FOR MY PERMANENT LICENSE?

We now use the Federation of State Medical Boards (FSMB) credentialing service. You will need The Uniform Application (UA) and The Federation Credentials Verification Service (FCVS)

Once these have been completed, you should receive an email from noreply@maine.gov with a pending license# and an Access Code.

You'll go to our MAIN PAGE and click License Options, then choose Submit Maine Application Addendum and click Continue.

Once you've entered your information and created a password, you'll be able to continue with the process of your application.
CHECK YOUR STATUS You can check your status online HERE using Check the Status of an Application. If you receive a message that says "No Possible Transactions", you should be all set!
WHEN WILL I BE APPROVED? We try to be sure that applications are fully processed within 60 days; however, it can take 90 or more days, depending on how quickly all requirements are met. Once completed, your application will go to the Board's next scheduled monthly meeting. The Board meets on the second Thursday of each month - please be sure to check the Administrative link to be sure nothing has changed.

Maine does not accept electronic notarizations. The applicant must be in the direct presence of the notary.

Applicants must use the Affidavit & Reference forms for this Board (reference letters are not accepted). Per the instructions on the UA Addendum, pre-filled forms and/or forms submitted with stamped or scanned signatures are not accepted; references must be submitted by the writer, not by the applicant or a third party.
- Once you have completed all requirements, your application will go to the Board's next regularly scheduled monthly meeting. The Board meets on the 2nd Thursday of each month. If you receive a message stating that there are no possible transactions available, your application requirements have been met and your application will be presented for review at the Board's next scheduled meeting.

TEMPORARY EDUCATION PERMITS:
Complete and submit the Uniform Application. Please be sure to check off for it to be sent to the Maine Board of Osteopathic Licensure. The FSMB charges a $60 fee to submit the application.
After submitting the UA, you'll receive an email from noreply@maine.gov which will have your pending license# and an Access Code.
Use the information in the email to complete the Maine Application Addendum on OUR SITE.
Complete a Release Affidavit and submit the original to either the Board office or your Program Coordinator (check with them for their preference).
Your training program will typically pay your application fee.
Please check with your Program Coordinator regarding the submission of your Uniform Application, online application fee and Affidavit.

The Maine Board of Osteopathic Licensure is responsible for ensuring the health and safety of the public of this State. It is the responsibility of the Board to determine which applicants qualify to practice osteopathic medicine in the State of Maine by regulating the practice of its licensees. Through its licensing process, the Board provides assurance that the physician has received his/her degree from an accredited college of osteopathic medicine, has received the appropriate level of training and that no cause exists that would be a basis for disciplinary action of a licensed physician under 32 M.R.S.A. sec 2591-A.
Once licensed, the physician must adhere to accepted standards of professional conduct. Further, once a license is granted, it must be renewed every two years. Each licensee must demonstrate that he/she has obtained the necessary continuing medical education. If grounds for discipline exist at the time of license renewal, the Board may take appropriate action allowed by 10 M.R.S.A. sec. 8003 and 32 M.R.S.A. sec. 2591-A.
