Disease Reporting
Disease reports from community practitioners and laboratorians constitute the basis for effective public health prevention and intervention efforts. We wish to thank the many physicians, physician assistants, infection control practitioners, clinical microbiologists, extended care facility nurses, administrators, nurse practitioners, and clinical office staff, among others, whose disease reporting efforts are critical to maintaining public health in Maine.

Electronic Case Reporting (eCR)
What is eCR? Electronic case reporting (eCR) is the automated, real-time exchange of case report information between electronic health records (EHRs) and public health agencies (PHAs) for review and action (disease tracking, case management, and contact tracing). Like traditional reporting, eCR captures critical patient and clinical data regarding demographics, comorbidities, immunizations, medications, and other treatments, but greatly reduces the burden of manual reporting. eCR also allows public health to provide information back to healthcare professionals.
Why eCR? Electronic case reporting (eCR) replaces antiquated manual reporting methods like faxing or mailing paper forms, using digital technology for automating disease reporting, resulting in faster disease detection, streamlined data collection, and the potential for more effective interventions. eCR enables healthcare providers to fulfill their mandated reporting requirements and report cases for further investigation from their EHR faster, more accurately, and more reliably than traditional, manual reporting. eCR also allows eligible professionals, eligible hospitals, and critical access hospitals to satisfy the Center for Medicare and Medicaid Services (CMS) regulatory requirements of the Public Health and Clinical Data Exchange objective for the Promoting Interoperability Program (PIP) and the Merit-based Incentive Payment System (MIPS).
Get Started with eCR:
- Email ecr@cdc.gov or visit Getting Started with eCR to learn more.
- Work with the eCR team to connect with your public health agency.
- Register your intent for eCR with the appropriate public health agency.
- Healthcare facilities that wish to participate in the CMS Promoting Interoperability Program (PIP) and Merit-Based Incentive Payment System (MIPS) (PDF) must communicate their intent to implement electronic case reporting (eCR) with their state public health agency.
- To register for eCR with Maine CDC, fill out this registration form.
- Partner with your EHR vendor to discuss options for implementing eCR.
- Get approval from your organization's leaders to implement eCR.
