Elevation Certificate
The Elevation Certificate is an important administrative tool of the National Flood Insurance Program (NFIP). It is to be used to provide elevation information necessary to ensure compliance with community floodplain management ordinances, to determine the proper insurance premium rate, and to support a request for a Letter of Map Amendment (LOMA) or Letter of Map Revision based on fill (LOMR-F).The Elevation Certificate is required in order to properly rate post-FIRM buildings, which are buildings constructed after publication of the Flood Insurance Rate Map (FIRM), located in flood insurance Zones A1-A30, AE, AH, A (with BFE), VE, V1-V30, V (with BFE), AR, AR/A, AR/AE, AR/A1-A30, AR/AH, and AR/AO. The Elevation Certificate is not required for pre-FIRM buildings unless the building is being rated under the optional post-FIRM flood insurance rules.

Floodproofing Certificate
The flood proofing of non-residential buildings may be permitted as an alternative to elevating to or above the Base Flood Elevation; however, a flood proofing design certification is required. This form must be used for that certification.

MT-EZ LOMA
This form should be used by an individual property owner to request that the Federal Emergency Management Agency remove the Special Flood Hazard Area (SFHA) designation from a single structure or a legally recorded parcel of land or portion thereof by issuing a Letter of Map Amendment. The SFHA is the area, shown on a NFIP map that will be inundated by the flood having a 1-percent chance of being equaled or exceeded in any given year 

MT-1 LOMA Form
The application forms and instructions included in the MT-1 package were designed to assist requesters (community officials, individual property owners, and others) in gathering the information that the Federal Emergency Management Agency (FEMA) needs to determine whether property (i.e., parcel[s] of land, structure[s]) is likely to be flooded during the flood event that has a 1-percent chance of being equaled or exceeded in any given year.

MT-2 LOMA Form
The application forms and instructions included in the MT-2 forms package were designed to assist requesters (community officials or individuals via community officials) in gathering the data that the Federal Emergency Management Agency (FEMA) needs to determine whether the effective National Flood Insurance Program(NFIP) map (i.e., Flood Hazard Boundary Map, Flood Insurance Rate Map, Flood Boundary and Floodway Map, or Digital Flood Insurance Rate Map) and Flood Insurance Study report for a community should be revised. These forms, which expire on December 31, 2010, also should be used by community officials or individuals via community officials for requesting FEMA comments on a proposed project issued in the form of a Conditional Letter Of Map Revision (CLOMR). These forms assure FEMA that all pertinent data relating to the request is included in the submittal. They also ensure that:

• The data and methodology are based on current conditions;
• Qualified professionals have assembled the data and performed necessary computations; and
• The individuals and organizations affected by proposed changes are aware of the changes and have an opportunity to comment.
