Honorable Service Certificate Application and Eligibility Criteria (PDF) This application is used to request Honorable Service Certificates for a specific period such as World War II, Korean War, Vietnam War, etc. Certificates issued for World War II, Korean War and Vietnam War have the likeness of the corresponding plaque imprinted on the certificate. The plaques hang in the Hall of Flags at the Capitol in honor of the men and women who served during those time periods.
