Program Enrollment
Maine's Universal Immunization Program provides federally purchased vaccines supplied by the Maine Immunization Program (MIP) at no cost to providers. Providers may charge an administration fee, however, no patient 0 to 64 may be denied vaccination due to an inability to pay.
Becoming a Provider
If you are interested in enrolling as provider, please email ImmunizeME.DHHS@maine.gov. After contacting the program, staff will guide you through the enrollment process and provide the required forms and information packet.
Enrollment Process
Once your completed paperwork has been received, program staff will schedule an initial virtual site visit with your office. This visit ensures that your staff are familiar with program requirements, have appropriate vaccine storage and handling resources, and understand documentation procedures.
Following the visit, your office will receive access to Maine's Immunization Information System (ImmPact IIS) and be scheduled for a virtual training session on how to use the system.
All enrollment forms and training sessions must be completed before vaccine orders can be placed.
Who is Eligible for State-Supplied Vaccine?
Maine's Universal Immunization Program provides vaccine to enrolled providers for most Maine residents ages 0 to 64.
Eligible:
- Maine residents ages 0 to 18 (regardless of insurance status)
- Maine residents 19 to 64 (underinsured/uninsured, privately insured, or enrolled in MaineCare)
- Adults 19 to 64 enrolled in Medicare for vaccines NOT covered by Medicare
Not Eligible:
- Adults age 65 and older
- Out of state residents
- 19 to 64 years
- Insured 0 to 18 years
- Adults 19 to 64 enrolled in Medicare for vaccines covered by Medicare.

Providers who have completed the enrollment process with Maine's Universal Immunization Program can order vaccines through the Immunization Information System (IIS) - ImmPact.
